Photizo Hire Terms & Conditions

1. The photizo unit is supplied on the understanding that it will be used only by the person(s) whose name
appears on the booking form and is not transferable between individuals.

2. The person hiring the unit will have discussed the use of a photizo machine with a medical practitioner
in charge of their care and will have received the practitioners approval.

3. Photizo will be used under medical supervision and guidance following the instructions provided.

4. The success of Photizo is neither guaranteed or implied.

5. The unit will be supplied to the best of our ability in perfect working order ready for use.

6. Photizo should not be used by anyone suffering from epilepsy, fitted with a cardiac pacemaker or who
suffers from (or is suspected of suffering from) heart disease.

7. Danetre Health Products Ltd (DHP) reserves the right to charge the replacement value of lost, stolen or
damaged Photizo units or part of a unit. The replacement cost may be charged if any Photizo unit or part of
a unit is returned contaminated with bodily fluids.

8. DHP shall not be held responsible or liable for any injury or damage sustained as a result of misusing our
Photizo unit.

9. The return of the Photizo unit to DHP rests with the hirer unless otherwise arranged with DHP. A
certificate of posting is to be obtained for all return packages and the receipt retained for a period of one
month as proof of posting.

10. The hire period is for a minimum of 1 week but recommended that used for 2 weeks to establish
benefit. DHP will take payment for additional hire by internet bank transfer or by cheque. If the Photizo is
not returned for any reason, all outstanding sums will be automatically charged to the customer’s credit or
debit card.

11. Extension of the hire period must be agreed with DHP. Any unauthorized extension of the hire period is
not permitted and may be subject to the additional charges.

12. All Photizo units are tested immediately prior to dispatch. However the liability of DHP shall not exceed
the hire charge.

13. DHP reserves the right to alter Terms and Conditions of hire without prior notice.
